Wegovy and semaglutide: one drug, different names
Semaglutide is the name of a medicine. Wegovy is a brand name for that medicine. Wegovy is made by Novo Nordisk, a drug company from Denmark. So when people ask about Wegovy vs. semaglutide, they are really asking about a brand version vs. the same drug sold another way.
Think of it like Tylenol and acetaminophen. Tylenol is the brand. Acetaminophen is the drug inside. Semaglutide also comes as Ozempic (for type 2 diabetes) and Rybelsus (a diabetes pill). All contain the same active ingredient. The difference is the dose, the form, and what the FDA (the U.S. Food and Drug Administration) approved each one to treat.
How semaglutide helps with weight loss
Semaglutide copies a gut hormone called GLP-1. Your body makes GLP-1 after you eat. It tells your brain you are full, slows down how fast your stomach empties, and helps control blood sugar. Semaglutide does the same things, but it lasts about a week instead of a few minutes.
The result is that most people feel full sooner and think about food less. They eat less without trying as hard. Over months, that adds up to real weight loss.
The biggest study of Wegovy for weight is called STEP 1. It was a randomized controlled trial published in the New England Journal of Medicine in 2021. Adults without diabetes who took the full 2.4 mg dose lost about 15% of their body weight over 68 weeks. People on a placebo (a fake shot) lost about 2.4%. Everyone also got diet and exercise coaching.
What makes Wegovy different from Ozempic
Wegovy and Ozempic are both semaglutide shots taken once a week. The main difference is the top dose. Wegovy goes up to 2.4 mg. Ozempic tops out at 2 mg. Wegovy is FDA-approved for weight management. Ozempic is FDA-approved for type 2 diabetes and heart protection in people with diabetes.
Doctors sometimes prescribe Ozempic for weight loss anyway. That is called off-label use, and it is legal. But insurance often will not pay for it when it is used that way.
Wegovy also has an extra approval. In 2024 the FDA said it can lower the risk of heart attack and stroke in adults with heart disease who are overweight or have obesity. That came from a large trial called SELECT.
What is compounded semaglutide?
Compounded semaglutide is semaglutide made by a licensed compounding pharmacy instead of by Novo Nordisk. A compounding pharmacy mixes medicines to order for a specific patient. The active ingredient is semaglutide, but the product is not Wegovy.
Here is the important part. Compounded semaglutide is not FDA-approved. The FDA does not review each batch for safety or strength the way it does for Wegovy. Compounding became common when Wegovy was on the FDA drug shortage list from 2022 into early 2025. The FDA has since said the shortage is over, and it has limited when pharmacies can make copies.
That does not make all compounded semaglutide unsafe. Licensed pharmacies that follow the rules can make quality products. But you should know what you are getting, where it was made, and that it does not carry the same FDA approval as the brand.
Cost: the biggest real-world difference
Wegovy's list price is over $1,000 a month without insurance. Many insurance plans do not cover weight-loss drugs at all. Novo Nordisk offers lower cash prices through its own programs, but the cost is still high for many families.
Compounded semaglutide usually costs far less. At Zen IV Spa, compounded semaglutide is $199 per month. That includes a free video visit with the medical director first, and labs drawn at home in New York and New Jersey by a licensed nurse.
Price matters, but it should not be the only thing you look at. Ask where the medicine is made, how it is shipped, and who is checking on you while you take it.
Which one is right for you?
If your insurance covers Wegovy and you want the FDA-approved brand, Wegovy is a clear choice. If you pay out of pocket, compounded semaglutide can make treatment possible when the brand is not.
Either way, the medicine works the same way inside your body. What changes the result most is the plan around it: steady dose increases, enough protein, some movement, and someone to call when side effects show up.
Semaglutide is not for everyone. People with a personal or family history of medullary thyroid cancer, or a condition called MEN 2, should not take it. Tell your prescriber about any history of pancreatitis, gallbladder problems, or diabetic eye disease.
