Your Rights Under HIPAA
You have the right to inspect and obtain a copy of your medical records, request corrections to your records, request an accounting of certain disclosures we have made, request restrictions on how we use or share your information, request confidential communications, and receive a paper copy of our Notice of Privacy Practices upon request. How We Safeguard Your Records
We maintain administrative, physical, and technical safeguards designed to protect your health information, including access controls limiting PHI to staff directly involved in your care, encrypted storage and transmission of electronic health records, and staff training on HIPAA requirements as a condition of employment.
Business Associates
Where we work with outside vendors — such as our electronic health record platform, payment processor, or laboratory partners — who may access PHI to support your care, we maintain Business Associate Agreements requiring them to protect your information to the same HIPAA standard we hold ourselves to.
Reporting a Concern
If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer using the contact information below, or with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.
