Acceptance of Terms
By using this website or booking a service with Zen IV Spa, you agree to these Terms of Service. If you do not agree, please do not use our website or services. These terms work alongside our Privacy Policy and Medical Disclaimer, both incorporated by reference.
Booking, Cancellation & Payment
Appointments can be booked through our website, by phone, or by text. Cancellations made at least four hours before your scheduled appointment are free of charge. Cancellations inside that window, or missed appointments, may be charged up to half the price of the visit; this fee is waived when you are unwell. Payment is due at the time of service unless you hold an active membership or approved corporate account.
Membership Terms
Memberships are billed monthly, carry no long-term contract, and may be paused or cancelled at any time effective at the end of the current billing cycle. Unused monthly IV credits roll forward for 90 days from the date they are issued and expire after that window. Member discounts and included credits apply only to the guest named on the membership, except where a plan explicitly permits sharing with a household or designated individual, as described on our Membership & Pricing page.
Eligibility & Medical Screening
All services are subject to a medical intake and screening by a licensed provider. We reserve the right to decline, modify, or discontinue any service where, in our clinical judgment, it is not appropriate for you, including based on your health history, current medications, or vital signs at the time of your visit. Prescription programs additionally require current lab work and provider approval before any medication is dispensed.
Website Use
You agree to use this website only for lawful purposes and not to misuse it, interfere with its operation, or attempt to access it using any method other than the interface we provide. Content on this website — including text, images, and pricing — is the property of Zen IV Spa or its licensors and may not be copied or republished without permission.
Pricing
Prices listed on our website are current as of the time of publication and are subject to change. The price confirmed at the time of booking, or communicated to you directly, will apply to your appointment. Member pricing requires an active membership in good standing.
Limitation of Liability
To the fullest extent permitted by law, Zen IV Spa's liability arising from your use of this website is limited to direct damages and shall not include indirect, incidental, or consequential damages. Nothing in these terms limits liability for clinical negligence, which is governed by applicable medical malpractice law rather than this website agreement.
Governing Law
These terms are governed by the laws of the State of New York, without regard to conflict-of-law principles, except where services are rendered in New Jersey, in which case New Jersey law governs the clinical relationship for that visit.
Changes to These Terms
We may revise these terms from time to time. The version posted on this page at the time of your appointment or website visit is the version that applies.
