Why Wegovy can slow things down

Wegovy is a brand-name weight-loss shot made by Novo Nordisk, a Danish drug company. The medicine inside it is called semaglutide. Semaglutide copies a gut hormone called GLP-1. One of the main things GLP-1 does is slow down how fast food moves through your stomach and intestines.

That slowing is part of why the drug helps you lose weight. You feel full longer and eat less. But the same effect can make stool move more slowly through the colon. The longer stool sits there, the more water the body pulls out of it. Dry, hard stool is harder to pass. That is constipation.

Many people also eat and drink less when they start the medicine. Less food means less fiber. Less fluid means drier stool. Both add to the problem.

How common is it?

In the main clinical trial for Wegovy, called STEP 1, about 24 out of every 100 people taking the drug reported constipation. In the group taking a placebo (a fake shot), about 11 out of 100 did. So the drug roughly doubles the chance of it.

The good news is that most cases were mild or moderate. Very few people stopped the medicine because of it. For most people, constipation shows up in the first weeks, or right after a dose increase, and then eases as the body gets used to the drug.

Simple fixes that help most people

Drink more water. Aim for a glass with every meal and a few in between. Many people on GLP-1 drugs forget to drink because they are not hungry or thirsty. Dehydration is the number one reason stool gets hard.

Add fiber slowly. Fruits, vegetables, oats, beans, and chia or flax seeds all help. If you add a lot of fiber at once you may feel bloated, so build up over a week or two. A fiber supplement like psyllium can help if you are not eating much.

Move your body. Even a 15-minute walk after a meal helps your intestines keep moving. Try to walk every day, especially in the weeks right after a dose change.

Do not skip meals on purpose. Eating small, regular meals gives your gut a reason to move. Going all day without food can make constipation worse.

When over-the-counter help makes sense

If water, fiber, and walking are not enough after a few days, a gentle over-the-counter product may help. Polyethylene glycol (sold as MiraLAX) pulls water into the stool and is safe for most people to use for a short time. Stool softeners like docusate can also help.

Stimulant laxatives such as senna or bisacodyl work faster but should not be used every day without talking to a clinician. Always read the label and follow the dose instructions.

Tell your prescriber what you are taking. They may want to hold your dose at the current level a bit longer before increasing it, which often gives your body time to adjust.

Signs you should call a clinician

Mild constipation is expected. Some symptoms are not. Call your care team right away if you have severe belly pain, a swollen or hard belly, vomiting, or no bowel movement for more than a week. These can be signs of a blockage, which is rare but serious.

Also reach out if you see blood in your stool, lose weight much faster than planned, or feel dizzy and dry-mouthed, which can mean you are dehydrated. These are easy to fix early and harder to fix late.

How Zen IV Spa handles this

At Zen IV Spa we prescribe compounded semaglutide, the same active ingredient found in Wegovy, for $199 per month. Compounded medicines are made by a licensed pharmacy for a specific patient. They are not FDA-approved products, and we tell every client that up front.

Every client starts with a free video visit with our medical director. A nurse comes to your home in New York and New Jersey to draw labs. We start at a low dose and move up slowly, which is the best-studied way to keep stomach and bowel side effects low. If constipation shows up, you message our team and we adjust the plan.

Common Questions

How long does Wegovy constipation last?+

For most people it is worst in the first few weeks or right after a dose increase, then improves over one to two months as the body adjusts. If it lasts longer, tell your prescriber.

Can I take MiraLAX with Wegovy?+

Yes, polyethylene glycol is generally safe to use short term with semaglutide. Check with your clinician if you need it for more than a week or two.

Does a lower dose cause less constipation?+

Usually yes. Side effects tend to rise with dose. Your prescriber may keep you at a lower dose longer so your body can adjust.

Will more fiber fix it on its own?+

Fiber helps, but only if you also drink enough water. Fiber without fluid can make things worse.

Is compounded semaglutide the same as Wegovy?+

It contains the same active ingredient, semaglutide, but it is made by a compounding pharmacy and is not an FDA-approved product.

Does Zen IV Spa help with side effects after I start?+

Yes. You can message our care team any time, and we adjust your dose schedule based on how you feel.
