What a maintenance dose means
Tirzepatide is the active ingredient in Zepbound and Mounjaro, made by Eli Lilly. The FDA label for Zepbound lists three maintenance doses: 5 mg, 10 mg and 15 mg, given once a week. A maintenance dose is simply the dose you stay on once you have climbed the ladder from the 2.5 mg starting dose.
The word maintenance confuses people. It does not mean a special lower dose you switch to after you lose weight. It means the ongoing dose. Many people stay on the same dose that got them to their goal. Zen IV Spa prescribes compounded tirzepatide, which is not FDA-approved, using the same framework.
What happens if you stop
The clearest answer comes from the SURMOUNT-4 trial, published in JAMA in 2024. Everyone took tirzepatide for 36 weeks and lost about 21 percent of their body weight. Then half kept taking the drug and half switched to placebo without knowing it.
Over the next year, the people who kept taking tirzepatide lost another 5 percent or so. The people who switched to placebo regained about 14 percent of their body weight. They did not go all the way back to the start, but they got most of the way there. Everyone also had lifestyle support the whole time.
This is not a failure of willpower. Obesity is a long-term condition. When the drug leaves your system, the hunger signals it was quieting come back. Blood pressure, cholesterol and blood sugar improvements also faded in the placebo group.
Option one: stay on your full dose
The simplest path is to keep taking the dose that worked. This has the most evidence behind it. Weight tends to level off after 12 to 18 months and then hold steady. Side effects usually fade long before that. For many people this is the right long-term plan, the same way someone with high blood pressure stays on their medication.
Option two: step down to a lower dose
Some clinicians try a lower maintenance dose once weight is stable. The idea is that less drug may be enough to keep hunger in check once you are no longer trying to lose. There is no large trial that proves this works for tirzepatide, so it is a judgment call rather than a proven method.
If you try it, go one step at a time and watch the scale for four to eight weeks. A small bounce of a few pounds is normal. A steady climb is a sign to go back up. Your prescriber should guide this, not a calendar.
Option three: spacing doses out
Another approach some people try is taking the same dose every 10 or 14 days instead of every 7. Tirzepatide has a half-life of about five days, so stretching the gap means the drug level dips lower before each shot. This is off-label and has not been studied in a controlled trial. Some people report it works for them. Others find hunger returns in the last few days of each cycle. If you want to try it, do it with your prescriber's knowledge.
What helps regardless of dose
Three habits predict who keeps weight off. Protein intake, which protects muscle. Strength training two or three times a week. And regular self-weighing so you catch a trend early. Sleep and stress management matter too, because both drive hunger.
Think of the medication as one leg of a stool. The other legs are food, movement and sleep. A stool with one leg falls over.

Common Questions
What is the maintenance dose of tirzepatide?+
The Zepbound label lists 5, 10 and 15 mg weekly as maintenance doses. Most people stay on the dose that got them to their goal.
Will I regain weight if I stop tirzepatide?+
Most people regain a large share of it. In the SURMOUNT-4 trial, people who stopped regained about 14 percent of body weight within a year.
Can I lower my dose once I reach my goal?+
Some prescribers try it. There is no large trial proving it works, so it should be done gradually with close monitoring.
Can I take tirzepatide every two weeks for maintenance?+
This is off-label and unstudied. Some people try it with their prescriber. Watch for hunger returning late in each cycle.
How long should I stay on tirzepatide?+
Obesity is a chronic condition, and the research supports long-term use. The right length is a decision between you and your prescriber.
