The One-Paragraph Difference
Semaglutide acts on one receptor, GLP-1. Tirzepatide acts on two, GLP-1 and GIP. In head-to-head trials tirzepatide produced more average weight loss. Semaglutide has been studied longer, including for heart outcomes, and costs less. Both are weekly subcutaneous injections, both are titrated up slowly, and both cause nausea early if you go too fast.
How Our Provider Decides
Starting point, goal, history and budget. Someone with a lot to lose, or who plateaued on semaglutide, often does better on tirzepatide. Someone with a modest goal, or who wants the lower price, usually starts on semaglutide. People with a history of pancreatitis, certain thyroid cancers or severe GI disease may be steered away from both.
The Program
Every program at Zen IV Spa is prescribed and overseen by our medical director, a licensed physician, after a consultation and baseline labs. A nurse comes to your home in New York or New Jersey for your first dose if you want one, and refills ship to your door.
Monthly check-ins are included and dose changes are made by message. Muscle-preservation support, hydration IVs for the nausea weeks, and a maintenance plan at the end are all things we can add.
Cost
Semaglutide is $199 a month and tirzepatide is $299 a month. Brand-name Wegovy, Ozempic, Zepbound and Mounjaro are available by prescription where supply and insurance allow. A 2-month supply is 10% off and a 3-month supply is 15% off. The provider consultation is $50. That price includes the peptide from an FDA-registered US 503A compounding pharmacy, supplies, overnight refrigerated shipping, the intake review and follow-ups.
Common Questions
Which one loses more weight?+
On average tirzepatide. Individually, it varies, and plenty of people do very well on semaglutide.
Can I switch?+
Yes. The provider manages the switch and the dose.
Do I have to stay on it forever?+
No. We plan the taper and maintenance from the start.
Will I feel sick?+
Nausea in the first weeks is common and dose-related. Going slowly is the whole trick.
