Why Ozempic causes nausea

Ozempic is a weekly shot from Novo Nordisk. Its active ingredient is semaglutide, the same drug in Wegovy. It copies a gut hormone called GLP-1, which tells your brain you are full and slows how fast your stomach empties.

When food sits in the stomach longer than usual, the stomach sends signals that your brain may read as nausea. The effect is strongest in the first days after an injection, when the drug level is highest, and after each dose increase.

How common and how long

In the STEP 1 trial of semaglutide for weight loss, about 44 out of 100 people reported nausea at some point, compared with about 16 out of 100 on placebo. Most cases were mild to moderate and short-lived. Nausea was most common during the first 20 weeks, when the dose was going up every four weeks.

After people reached their full dose and stayed there, new nausea became much less common. Only about 1 in 20 people stopped the drug because of a stomach side effect.

Food changes that make the biggest difference

Eat less at each sitting. On semaglutide, your stomach cannot handle the portions it used to. Try half-size meals and a snack in between if you need it.

Eat slowly and stop early. Fullness arrives later than usual, so by the time you feel it you may have already overdone it. Put your fork down when you feel 70 percent full.

Avoid fried, greasy, and very sweet foods. These are the most common triggers people report. Spicy food bothers some people too.

Lean on bland foods when you feel rough. Crackers, toast, rice, plain chicken, bananas, applesauce, and broth are easy on a slow stomach.

Try ginger. Ginger tea, ginger chews, or a small amount of fresh ginger in hot water has some evidence for easing nausea and is safe for most people. Peppermint tea helps some people as well.

Drinking, timing, and movement

Sip, do not gulp. Small amounts of cold water or an electrolyte drink through the day keep you hydrated without stretching the stomach. Avoid drinking a lot of liquid with meals.

Stay upright after eating. Lying down soon after a meal makes reflux and nausea worse. A slow walk after dinner helps the stomach empty.

Pick an injection day and time that fits your life. Many people inject in the evening, so the first-day peak happens overnight. Others pick a day before a quiet weekend.

Get enough sleep. Being overtired makes nausea feel worse. This is a small thing that a lot of people overlook.

Talk to your prescriber about the dose

The standard plan raises the dose every four weeks. But the FDA label allows staying at a lower dose longer if side effects are a problem. Many people do well staying at 0.5 mg or 1 mg for an extra month or two before moving up.

If nausea is severe, a short course of an anti-nausea medicine like ondansetron is sometimes used. Do not take leftover prescriptions from someone else. Ask your clinician.

Seek care right away for vomiting that lasts more than a day, inability to keep fluids down, or severe belly pain that spreads to the back. These can signal dehydration or pancreatitis, which need prompt treatment.

Common Questions

How long does Ozempic nausea last?+

It usually peaks in the first one to three days after a shot and fades over a few weeks at each dose. Most people have far less nausea once they reach a steady dose.

What should I eat when I feel nauseous on Ozempic?+

Small amounts of bland food: crackers, toast, rice, bananas, applesauce, broth. Ginger tea can help too.

Does nausea mean the dose is too high?+

Maybe. If nausea is more than mild, ask your prescriber about staying at your current dose longer before increasing.

Can I take Zofran with Ozempic?+

Some prescribers use ondansetron (Zofran) short term. Ask your clinician rather than using someone else's prescription.

Will the nausea come back with every dose increase?+

Often there is a short bump with each increase, but it is usually milder than the first time and fades faster.

What if I cannot keep water down?+

Call your care team the same day. Dehydration can get serious quickly, and you may need fluids or a dose change.
