What the research says about regain

GLP-1 medicines like semaglutide (in Wegovy and Ozempic) and tirzepatide (in Zepbound and Mounjaro) work very well while you take them. The question is what happens after.

The clearest answer comes from two extension studies. In STEP 1, people who stopped semaglutide after 68 weeks regained about two-thirds of the weight they had lost within one year. In SURMOUNT-4, people who switched from tirzepatide to placebo regained about half of what they had lost over the next year, while those who stayed on the drug kept losing.

These are randomized trials, the strongest kind of evidence. They tell us that for most people, obesity behaves like a long-term condition. The medicine controls it rather than cures it, much like blood pressure pills.

Why the body pushes weight back on

When you lose weight, your body fights back. Hunger hormones like ghrelin go up. Fullness hormones go down. Your metabolism slows a bit more than the weight loss alone would predict. Scientists call this metabolic adaptation.

GLP-1 drugs quiet those hunger signals. When the drug stops, the signals come back at full volume while your body is still smaller and burning fewer calories. That is a tough combination, and it is why willpower alone usually loses.

Option one: stay on a maintenance dose

The simplest way to prevent regain is not to stop. Many people move to a lower dose or a less frequent shot once they reach their goal. There is early evidence that a lower maintenance dose keeps most of the benefit for many people, though this is still being studied.

Cost is often the reason people stop. Brand drugs can cost well over $1,000 a month without insurance. Compounded versions, which are made by a licensed pharmacy and are not FDA-approved, cost far less. At Zen IV Spa, compounded semaglutide is $199 per month and compounded tirzepatide is $299 per month.

Option two: taper slowly instead of stopping cold

If you do want to come off, a slow taper gives your body time to adjust. Some clinicians lower the dose every four to eight weeks over several months rather than stopping all at once. This has less formal study behind it, but many people find the hunger returns more gently.

During a taper, this is the time to build the habits below so they are solid before the drug is fully gone.

Habits with the best evidence for keeping weight off

Protein at every meal. Aim for roughly 0.7 to 1 gram per pound of goal body weight each day. Protein is the most filling nutrient and protects muscle.

Strength training two to three times a week. Muscle burns calories at rest, and keeping it is the best defense against a slower metabolism.

Daily movement. People in the National Weight Control Registry, a long-running study of people who kept off 30 pounds or more for years, average about an hour of activity a day, mostly walking.

Weigh yourself regularly. The same registry found that people who weigh in weekly or daily catch small gains early, while they are easy to reverse.

Keep the fiber and water habits you built on the drug. Fiber-rich meals keep you full for far fewer calories.

Sleep seven to nine hours. Short sleep raises hunger hormones the next day. This is one of the most overlooked tools.

Keeping it off with Zen IV Spa

Whether you want to stay on a low maintenance dose, taper slowly, or come off completely with a plan, our medical director will build it with you in a free video visit. A nurse draws labs at your home in New York and New Jersey so we can track your metabolic health, not just your weight.

We prescribe compounded semaglutide ($199 per month), compounded tirzepatide ($299 per month), and retatrutide ($249 per month). All are compounded, which means they are made by a licensed pharmacy and are not FDA-approved. Learn more at /weight-loss/ or call (332) 239-2005.

Common Questions

How fast do people regain weight after stopping semaglutide?+

In the STEP 1 extension, people regained about two-thirds of their lost weight within one year of stopping.

Is it the same with tirzepatide?+

Similar. In SURMOUNT-4, people who switched to placebo regained about half of their lost weight in a year, while those who stayed on the drug kept losing.

Can I stay on a lower dose forever?+

Many people do. Obesity is treated as a long-term condition, like high blood pressure. Your clinician can help you find the lowest dose that holds your weight.

Does tapering work better than stopping suddenly?+

There is less research on tapering, but many clinicians use it, and people often report hunger returns more gradually.

What is the single most important habit for keeping weight off?+

There is no single one, but daily activity and regular self-weighing have the strongest long-term evidence from the National Weight Control Registry.

Is compounded semaglutide a good option for maintenance?+

It contains the same active ingredient as Wegovy at a lower cost. It is made by a compounding pharmacy and is not FDA-approved, which we explain to every client.
