Yes, it is physically possible
Erection and ejaculation are two separate events run by two separate systems. An erection happens when blood flows into the penis and gets trapped there. Ejaculation happens when nerves trigger muscles at the base of the penis to push semen out. You can have one without the other.
A soft-penis climax, sometimes called a flaccid ejaculation, happens when the ejaculation system is working fine but the blood-flow system is not. Men often describe it as reaching orgasm through manual or partner stimulation while the penis stays soft or only partly firm.
This is not dangerous, and it does not mean something is wrong with your fertility or your hormones in most cases. But it is a clear sign of erectile dysfunction (ED), and ED is worth checking because it can be an early warning for heart and blood-vessel health.
Why the erection part fails
The most common cause is reduced blood flow. Conditions that narrow or stiffen arteries, like high blood pressure, high cholesterol, diabetes, smoking, and obesity, all reduce the blood that reaches the penis. The arteries in the penis are small, so they show trouble years before the heart does.
Nerve problems are another cause. Diabetes, prostate surgery, spinal injury, and multiple sclerosis can all disrupt the signal that tells blood vessels to open. The nerves for ejaculation often survive when the nerves for erection are damaged, which explains why climax still happens.
Medicines matter too. Some blood pressure drugs, antidepressants, antihistamines, and prostate medicines can make erections harder to get while leaving ejaculation intact.
The psychological piece
Stress, anxiety, depression, and relationship conflict can shut down erections even when the plumbing is fine. Performance anxiety is a loop: one soft episode causes worry, and worry makes the next episode more likely.
A clue to a psychological cause is whether you still get firm erections at other times, like in the morning or with masturbation. If you do, the hardware works and the issue is more likely in the mind. If you never get firm erections anywhere, a physical cause is more likely.
Both kinds respond to treatment. Many men have a mix of the two.
Low testosterone and other hormone issues
Testosterone drives desire and helps maintain the tissue of the penis. Very low levels can make erections weaker and less frequent, though orgasm can still happen. Low testosterone often shows up alongside fatigue, low mood, less muscle, and more belly fat.
Thyroid problems and high prolactin, a pituitary hormone, can also affect erections. A simple blood panel checks all of these.
A Zen IV nurse can draw these labs at your home in New York and New Jersey so you can get answers without a clinic visit.
What treatment looks like
For most men, the first step is a PDE5 inhibitor. Sildenafil, the generic form of Viagra, and tadalafil, the generic form of Cialis, increase blood flow to the penis during arousal. In clinical trials they help about 60 to 80 percent of men with ED, including many with diabetes or nerve damage.
Fixing the underlying cause matters as much. Lowering blood pressure and cholesterol, controlling blood sugar, losing weight, quitting smoking, and exercising all improve erections over time. Switching a medicine that blocks erections can help quickly.
If anxiety is part of it, short-term counseling or sex therapy works well, especially when combined with a pill that gives you a few successful experiences to rebuild confidence.
When to see a clinician right away
See a doctor soon if ED came on suddenly, if you also have chest pain or shortness of breath, if you have a curve or lump in the penis, or if you have symptoms of low testosterone like fatigue and loss of body hair.
ED is often the first sign of blood-vessel disease. Men with ED have a higher risk of heart attack and stroke in the years that follow, so treating it is a chance to catch bigger problems early.
At Zen IV Spa, our ED program is $129 and includes a video visit with the medical director and a prescription for generic sildenafil or tadalafil if appropriate, shipped to your door. If you also struggle with ejaculating too fast, our PE program is $79.

Common Questions
Can a man ejaculate without being hard?+
Yes. Erection and ejaculation use different nerves and systems. A soft climax usually means a blood-flow or nerve problem with the erection, not with ejaculation.
Does this mean I am infertile?+
Not by itself. Sperm production and ejaculation are separate from erection. A semen analysis can confirm fertility if you are concerned.
Is it a sign of low testosterone?+
It can be, but most cases are from blood flow, nerves, medicines, or anxiety. A morning testosterone test gives a clear answer.
Will Viagra or Cialis help?+
For most men, yes. These medicines increase blood flow during arousal and work in 60 to 80 percent of men with ED in clinical trials.
Should I worry about my heart?+
ED is linked with a higher risk of heart disease later. It is worth having blood pressure, cholesterol, and blood sugar checked.
