What Zepbound is
Zepbound is a weekly shot made by Eli Lilly, a drug company. Its active ingredient is tirzepatide. Tirzepatide acts on two gut hormones, GLP-1 and GIP. It makes you feel full sooner and slows your stomach. Zepbound is FDA-approved for weight loss and for sleep apnea in people with obesity.
Zen IV Spa prescribes compounded tirzepatide for $299 a month. Compounded medicines are made by a licensed pharmacy and are not FDA-approved. They use the same active ingredient as the brand.
Menstrual changes are not listed as a side effect on the Zepbound label. But many women report them, and there are some good reasons why.
Weight loss changes hormones
Fat tissue is not just storage. It makes estrogen. When you lose a lot of fat, your estrogen levels shift. That can change the timing, heaviness, or length of your period.
This is most clear in women with polycystic ovary syndrome, or PCOS. PCOS is a common condition with irregular periods, higher male-type hormones, and often weight gain. Losing even 5 to 10 percent of body weight can bring periods back on a more regular schedule. Small studies of GLP-1 medicines in women with PCOS found more regular cycles after weight loss.
For women who had regular periods before, the first few months can bring a cycle that is early, late, lighter, or heavier. This usually settles as weight stabilizes.
Nausea, eating less, and stress
Zepbound often causes nausea and a much smaller appetite in the first weeks. Eating very little, especially too little fat and protein, can push the body into a stress state. The brain may then delay ovulation, which delays your period.
Rapid weight loss itself is a stress signal. Some women skip a period during fast drops. If you are eating under about 1,200 calories a day without a doctor's guidance, that is worth fixing.
Poor sleep, which can happen with new medicine and life changes, also plays a role. Cycle changes are often a mix of all of these rather than one cause.
Birth control pills work less well
This is the most important point in this article. Zepbound slows how fast your stomach empties. That can lower how much of an oral birth control pill your body absorbs, mainly after you start and for about four weeks after each dose increase.
The FDA label tells women on birth control pills to switch to a non-oral method, or add a backup method like condoms, for four weeks after starting and after each dose change. Non-oral options include the patch, ring, shot, implant, or IUD.
This matters because weight loss can also raise fertility. Women who had trouble getting pregnant sometimes conceive faster after losing weight. If a pregnancy would be unwanted or unplanned, use a reliable method.
When to call a doctor
Call your prescriber if your period is more than three months late and you are not pregnant, if bleeding is very heavy, soaking a pad an hour, or if you have severe pelvic pain. These are not typical GLP-1 effects and need a check.
Take a pregnancy test if you have missed a period and could be pregnant. Zepbound should be stopped at least two months before trying to conceive and is not used during pregnancy.
Keep a simple log of your cycle dates and any symptoms. It helps your prescriber see patterns fast.
