What 'maintenance dose' means

Wegovy is a weekly semaglutide shot made by Novo Nordisk. The maintenance dose is the dose you stay on after you finish climbing the dose ladder. For most people, that is 2.4 mg a week. Some people stay at 1.7 mg.

The word maintenance can be confusing. It does not mean you stop losing weight. It means you stop raising the dose. Your body is still getting the full effect of the medicine every week.

Most people reach the maintenance dose around month four or five. A lot of the weight loss happens after that.

What the trials show

In the STEP 1 trial, adults reached the 2.4 mg dose by week 16. But weight kept dropping all the way to about week 60. By week 68, the average loss was about 15% of body weight. So most of the loss happened on the maintenance dose, not during the climb.

Another trial, STEP 4, is even more telling. Everyone took Wegovy for 20 weeks and reached 2.4 mg. Then half kept taking it and half switched to a placebo without knowing. Over the next 48 weeks, the people who stayed on Wegovy lost about 8% more. The people who switched to placebo gained back about 7%.

That study makes two things clear. The maintenance dose keeps working. And stopping the drug usually brings the weight back.

When does weight loss slow down?

Almost everyone hits a plateau at some point. In the trials, the curve flattens out around 12 to 18 months. After that, the medicine is mostly holding weight steady rather than pushing it lower.

A plateau is not the drug failing. Your body burns fewer calories at a lower weight, and your appetite adjusts. Hunger slowly creeps back for some people even on the full dose.

If you plateau early, before six months, talk to your prescriber. Eating habits, sleep, alcohol, and other medicines can all slow things down.

It also helps to measure more than the scale. Waist size, how your clothes fit, blood pressure, and blood sugar often keep improving even when weight is flat. Those are real wins.

How to get the most out of the maintenance phase

Protein first. Aim for protein at every meal. It protects muscle while you lose fat and keeps you full longer.

Move your body. Walking and strength training two to three times a week help you keep muscle and keep the weight off. You do not need a gym.

Keep your dose steady. Skipping shots lets hunger return and can bring back nausea when you restart. If you miss a dose by more than five days, ask your prescriber how to restart.

Do you have to stay on it forever?

Obesity is treated as a long-term condition, like high blood pressure. The trials show that most people regain much of the weight within a year of stopping. So many prescribers plan for long-term use.

Some people do try a lower dose or a longer gap between shots once they reach their goal. There is not much research on this yet, so it should be done with a prescriber, not on your own.

What keeps weight off after stopping is the habits you built while on the drug. The medicine makes those habits easier to learn.

Common Questions

Does weight loss stop once I reach the maintenance dose?+

No. In STEP 1, most of the weight loss happened after people reached 2.4 mg, with the curve leveling off around week 60.

How much more weight did people lose on the maintenance dose?+

In STEP 4, people who stayed on Wegovy lost about 8% more over 48 weeks after reaching the full dose.

What happens if I stop Wegovy?+

Most people regain weight. In STEP 4, those who switched to placebo gained back about 7% in 48 weeks.

Why did my weight plateau?+

A plateau is normal, usually after 12 to 18 months. Your body adjusts and hunger slowly returns. Diet, sleep, and activity can help.

Can I stay at 1.7 mg instead of 2.4 mg?+

Yes. Both are FDA-approved maintenance doses. Your prescriber will decide based on results and side effects.
