Wegovy and its dose ladder

Wegovy is a weekly semaglutide injection made by Novo Nordisk and approved by the FDA for weight management. The label lays out a ladder: 0.25 mg for four weeks, then 0.5 mg, 1 mg, 1.7 mg, and finally 2.4 mg, each step lasting four weeks. The target maintenance dose is 2.4 mg.

Zen IV Spa prescribes compounded semaglutide, made by a licensed compounding pharmacy. It is not FDA-approved, but our medical director uses the same ladder as a guide. Compounded semaglutide comes in a vial, so smaller dose steps are possible.

What the label says about lower doses

The Wegovy label is more flexible than many people think. It says that if a patient cannot tolerate the 2.4 mg dose, the dose can be lowered to 1.7 mg for about four weeks and then increased again. In a 2024 update, the label added that patients who do not tolerate 2.4 mg can stay on 1.7 mg as a maintenance dose.

So yes, staying on 1.7 mg is allowed by the label. Staying on doses below that is outside the label but is still common in practice, especially for people who are losing well and want fewer side effects.

Do lower doses work?

The best data comes from the STEP trials, the large studies that led to Wegovy's approval. In STEP 1, published in the New England Journal of Medicine in 2021, people on 2.4 mg lost about 15 percent of their body weight over 68 weeks. People on placebo lost about 2.4 percent.

Lower doses were studied in an earlier phase 2 trial. In that study, 0.4 mg daily, which is roughly similar to 2.4 mg weekly, gave the most weight loss, and smaller daily doses gave less. The pattern is clear: more drug, more weight loss, on average. But averages hide a lot. Some people respond strongly to 1 mg. Others need the full dose.

Ozempic, the diabetes version of semaglutide, tops out at 2 mg. People taking it for diabetes often lose a meaningful amount of weight at 1 mg. That tells us lower doses are not useless for weight.

Reasons to stay on a lower dose

The most common reason is side effects. Nausea, fatigue, reflux and constipation often get worse at each step up. If you are losing weight steadily at 1 mg and feel fine, there is no rule that says you must climb. Many clinicians hold a dose for as long as weight is still dropping.

A second reason is reaching your goal. Once you are at a healthy weight, the question becomes how much drug you need to stay there. STEP 4, published in JAMA in 2021, showed that people who switched from 2.4 mg to placebo regained about two thirds of what they lost within a year. A lower dose may be a middle path between full dose and nothing, though it has not been tested in a large trial.

Cost and supply are a third reason. With compounded semaglutide at $199 a month from Zen IV Spa, cost is less of a barrier, but some people still prefer to use the least drug that works.

Reasons to go up

If the scale has stalled for six to eight weeks and hunger is back, a higher dose is the most proven next step. Weight loss on semaglutide is dose-dependent, and plateaus are common at middle doses. Going up is not failure. It is how the medication was designed to be used.

Also consider your health goals beyond the scale. Semaglutide at 2.4 mg reduced heart attacks and strokes in the SELECT trial, published in 2023. If heart protection matters to you, that evidence is at the full dose.

Talking it through with Zen IV Spa

Dose decisions should be made with your prescriber, not alone. Our medical director meets you on a free video visit, a nurse draws labs at your home in New York and New Jersey, and your compounded semaglutide ships to your door. We adjust the dose based on your results and how you feel, not a fixed script. Visit /weight-loss/ or call (332) 239-2005 to get started.

Common Questions

Is it okay to stay on 1 mg of Wegovy?+

It is outside the label but common in practice. If you are losing weight and tolerating it well, many prescribers hold the dose.

Does the Wegovy label allow a lower maintenance dose?+

Yes. The label allows 1.7 mg as a maintenance dose for people who cannot tolerate 2.4 mg.

Will I lose less weight on a low dose?+

On average, yes. Trials show more weight loss at higher doses. Individual responses vary a lot.

Can I lower my dose after reaching my goal?+

Some people do. It has not been proven in a large trial, so watch your weight closely and adjust with your prescriber.

Is compounded semaglutide dosed the same way?+

Zen IV Spa follows the same ladder as a guide. Because it comes in a vial, smaller adjustments are possible. It is not FDA-approved.
