What Zepbound is

Zepbound is a weekly injection made by the drug company Eli Lilly. Its active ingredient is tirzepatide. The FDA approved it in 2023 for weight management in adults with obesity, or with overweight plus a weight-related condition. Tirzepatide works on two gut hormone receptors, GLP-1 and GIP, to lower appetite and slow digestion.

Zen IV Spa prescribes compounded tirzepatide, which contains the same active ingredient but is made by a licensed compounding pharmacy. Compounded tirzepatide is not FDA-approved. The dosing ladder our medical director uses follows the same pattern as the Zepbound label.

The official starting dose is 2.5 mg

The Zepbound prescribing information is clear. You start at 2.5 mg once a week for four weeks. Then you move to 5 mg. After that, the dose can go up by 2.5 mg at a time, with at least four weeks at each step. The maintenance doses are 5 mg, 10 mg and 15 mg. The highest dose is 15 mg.

The label calls 2.5 mg a starting dose, not a treatment dose. It is not expected to produce much weight loss. Its job is to let your body get used to the drug.

Why the low dose exists

Tirzepatide slows the speed at which food leaves your stomach. If you start high, that effect hits all at once. The result is nausea, vomiting, diarrhea and sometimes dehydration. In the SURMOUNT-1 trial, published in the New England Journal of Medicine in 2022, the most common side effects were stomach related, and they were most common during dose increases.

Starting low and going slow gives your gut time to adapt. People who ramp gradually tend to stay on the medication. People who quit in the first month usually quit because of side effects. The 2.5 mg month is there to protect you from that.

There is also a safety reason. A sudden drop in food intake can cause dizziness, low blood sugar in people on diabetes drugs, and gallbladder problems from very fast weight loss. A slow start lowers these risks.

When a prescriber might skip 2.5 mg

There are a few situations where a clinician may start at 5 mg or higher. The most common is switching from another GLP-1 drug. If you have been on Wegovy at 2.4 mg or Mounjaro at a steady dose for months, your body is already used to this class of medication. Your prescriber may pick a tirzepatide dose that roughly matches where you were.

Another case is restarting after a short break. If you missed two or three weeks, the label suggests going back to your previous dose. If you have been off for a month or more, most clinicians restart lower.

Starting at 5 mg because you want faster results is not a good reason. The extra month saves little time and raises your odds of quitting.

What happens if you jump too fast

The main risk is side effects strong enough to make you stop. Severe nausea, vomiting and dehydration can land people in urgent care. Rapid weight loss also raises the chance of gallstones and can cause more muscle loss than slow steady loss.

If side effects hit hard at any dose, you do not have to push through. Your prescriber can hold you at the current dose longer, or drop back one step. There is no rule that says you must reach 15 mg. In SURMOUNT-1, the 5 mg group lost about 15 percent of body weight over 72 weeks, which is a strong result on its own.

How Zen IV Spa handles dosing

Our medical director sees every client on a free video visit first. A nurse comes to your home in New York and New Jersey to draw labs. If compounded tirzepatide is right for you, we ship it to your door for $299 a month and follow the standard dose ladder unless there is a medical reason to adjust it. You can message the team any time about side effects, and dose changes are made with you, not to you.

Common Questions

Is 2.5 mg of Zepbound a real dose?+

The label calls it a starting dose. It is meant to prepare your body, not to cause major weight loss. Some people do lose a little at this dose.

How long do I stay on 2.5 mg?+

Four weeks, according to the FDA label. Then you move to 5 mg if you are tolerating it.

Can I start at 5 mg if I was on Wegovy?+

Possibly. Prescribers often choose a matching dose when switching from another GLP-1. That decision depends on your dose and how long you took it.

What if 5 mg is too strong?+

Tell your prescriber. You can go back to 2.5 mg or stay at 5 mg longer. There is no rush.

Is compounded tirzepatide the same as Zepbound?+

It has the same active ingredient but is made by a compounding pharmacy and is not FDA-approved. Zen IV Spa uses licensed pharmacies and the same dose schedule.
