The short answer
Constipation can make you weigh more on the scale, but it does not make you gain fat. Stool has weight. If several days of stool are sitting in your colon, that weight shows up when you step on the scale. Once you have a normal bowel movement, it goes away.
How much are we talking about? Usually one to three pounds. Some people with severe constipation may hold more. That is a real number on the scale, but it is not real weight gain in the sense of body fat.
Why constipation feels like weight gain
Stool that sits in the colon ferments. Gut bacteria break it down and make gas. The gas stretches the intestines, which makes your belly feel tight and look bigger. Your pants fit worse. It is easy to read that as gaining weight.
Constipation also makes a lot of people feel sluggish and tired. When you feel heavy and slow, you move less. Less movement over weeks can lead to actual weight gain. So while constipation does not directly add fat, it can push you toward habits that do.
Things that cause both constipation and weight gain
Sometimes constipation and weight gain show up together because they share a cause. A low-fiber diet of processed food is the most common one. Processed food is high in calories and low in fiber, so it adds weight and slows the gut at the same time.
Not moving enough does both as well. Exercise stimulates the intestines and burns calories. Sitting all day does neither.
An underactive thyroid (hypothyroidism) slows your whole body, including the gut and your metabolism. If you have constipation, weight gain, feeling cold, and tiredness together, a simple blood test can check your thyroid.
Some medicines cause both. Certain antidepressants, pain medicines, and blood pressure drugs slow the gut and may add weight. Ask your prescriber if you suspect this.
Constipation on weight-loss medicines
This one surprises people. GLP-1 drugs like semaglutide and tirzepatide help you lose weight but also slow the gut. Constipation is a common side effect. In the Wegovy trial, about a quarter of people reported it.
If you are on one of these drugs and the scale stalls or bumps up for a few days, constipation is a likely reason. Fix the constipation and the number usually drops again. This is not a sign the drug has stopped working.
How to get regular again
Fiber first. Aim for 25 to 38 grams a day from beans, vegetables, fruit, oats, and seeds. Add it slowly over a week or two.
Water next. Fiber without water makes things worse. Aim for enough that your urine is pale yellow.
Move every day. A 20-minute walk is enough to stimulate your gut. Try it after a meal.
Do not ignore the urge. Holding it in trains your colon to hold it longer. Go when you need to.
If diet and movement are not enough after a week, an over-the-counter option like polyethylene glycol (MiraLAX) is safe for most people short term. See a clinician if constipation lasts more than three weeks, or if you have blood in your stool, severe pain, or unexplained weight loss.

Common Questions
How much weight can constipation add?+
Usually one to three pounds of stool weight. It goes away after a normal bowel movement and is not body fat.
Does constipation slow my metabolism?+
No. It does not change how many calories you burn. But feeling sluggish can make you move less, which does matter over time.
Why does my belly look bigger when I am constipated?+
Trapped stool ferments and makes gas, which stretches the intestines and makes your belly look and feel bloated.
Can my thyroid cause both constipation and weight gain?+
Yes. An underactive thyroid slows both the gut and metabolism. A blood test can check it.
Why am I constipated on semaglutide or tirzepatide?+
These drugs slow the gut as part of how they work. Fiber, water, and daily walks are the standard fix.
When should I see a doctor about constipation?+
If it lasts more than three weeks, or if you have blood in your stool, severe pain, vomiting, or weight loss you did not plan.
