What It Does
Lowers the liver's sugar output and improves insulin sensitivity. Mildly lowers appetite. Approved for type 2 diabetes; used off-label for prediabetes, PCOS and some weight goals.
Results
Two to three percent of body weight over a year in most studies. Better results in people with insulin resistance or PCOS. Not comparable to the 15 to 20 percent from GLP-1s.
Side Effects
Stomach upset and diarrhea early (extended-release helps). B12 depletion over years (we check it). Not for people with poor kidney function.
Where It Fits
Prediabetes or PCOS with modest weight goals. Alongside a GLP-1 for blood sugar. A low-cost maintenance tool after a GLP-1. We prescribe it after labs at your home.
